Summary

decisions.jsonl is append-only, one JSON object per line, UTF-8, no trailing commas. Records are never edited; a changed decision is a new record that supersedes the old one by id.¹

Field rules

• id is YYYY-MM-DD:<slug> and unique.
• actor follows the OKF convention: agent/<name>, human:<id>, process:<id>.
• evidence holds ledger ids or bundle-rooted paths, so a decision can be traced to what it rested on.
• status: superseded requires supersedes to name the replacement.
